In compliance with the constitution and canons of the diocese, this form is to be completed by the rector, or the clerk of the vestry, or the two wardens.

The Ecclesiastical Authority (Standing Committee) has called for a Special Convention on May 4, 2024, to be held concurrently with the Episcopal Dioceses of Eau Claire and Fond du Lac to consider a resolution to reunify the three dioceses in Wisconsin into one Episcopal Diocese of Wisconsin. Following our constitution (Article IV), the Ecclesiastical Authority is directing the election of lay deputies to the Special Convention.

The Annual Convention will take place in October 2024.
