New Employees
Below are the listings of plans that are available to employees of the Diocese of Milwaukee. Each plan has Vision, Prescription benefits and Mental health benefits attached. The HMO is only available in the Madison area and use of the HMO is limited to the Group Health Clinics. Coverage provided by the Plans not including Group Health is generally the same. The differences will be in the Provider Network and Hospitals and Clinics they contract with. The Network access instructions give you websites to go to to find out if your provider is a provider for that plan.
New employees have 30 days from the day they start to enroll in health insurance. A letter from the parish should indicate the employees full name, type of insurance, level of coverage and effective date and should be faxed, emailed or mailed to Patty at Nicholson House 804 E. Please review the documents below to decide on health coverage and to reveiw premium costs. The Plans offered are;
Diocesan Health Insurance Standards The health and dental insurance is administered by the national church Medical Trust. It is available to all parishes, their staff and clergy. The national guidelines are that insurance must be offered to all employees working 20 hours or more a week. The health plan includes drug, vision and mental health coverage. The dental plan is a separate plan that can be taken out by itself or with the health plan. Parishes are not obligated to offer these plans they may go outside the Diocesan offerings and find their own policy.
Life InsuranceThe diocese has a life insurance/accidental death/disability and Critical Care policy. This is One entire Policy you cannot choose to have just portions. The premium is determined by an equation used by Unum and it does indicate that in the forms below. Clergy that are working full time are expected to sign up for this benefit.
